Punctuating thinking out loud

At an English Teachers Teaching The A* Students Meeting we wondered: do any languages punctuate 'direct thinking' differently to 'direct speaking' (with pen and ink - so italics difficult)?
I thought: "Hello? Hello?"
I said: "Hello? Hello?"
We do in comics, and have a 'thinks' bubble. But when we write prose ...
