Planning / Stating Lesson Aims

I'm wondering how many qualified teachers still plan their lessons as they were trained to? How detailed do you make your plans, and how specific are the aims you state.
What I'm really interested in is how many teachers state their aims for the lesson / activity to students. Do you present a kind of menu of "what we'll be doing today and why" and then "what we've done today" in your lessons? Are there reasons why you do / don't do this?
