Help in one to one teaching!

I have a student in a one to one class who has to pass exams every two weeks or so (B1) and then he has a University entry exam next May.  He has a very low level of English in all the skills and even gets the basic structures wrong.  I find it difficult to start just from the basics because that wouldn't help him pass his college exams.  I think I should concentrate on boosting his vocabulary (also very poor) and "revise" basic grammar, but I can't fit all the skills into one hour lesson so that he actually benefits.  Although I prepare the lessons to try to follow some structure he might ask me to go through something that he has done in class and didn't understand so I find it extremely difficult to give any structure or continuity to the class.  I find it really frustrating because I don't think he is getting anything out of the class and because I don't think he is going to pass any exam at this rate. It is a while since I have done any teaching and really the first time I have a one to one student.  I would appreciate any tips to get this going and make any sense.  Any way of boosting vocabulary, listening or writing (as these are the main skills that are being evalutated).

      Thanks very much to everyone.
